Treatment Philosophy

Community Partnership Care Coordination’s mission is to provide timely, customized services to individuals of any age, using innovative health care approaches to foster engagement, improve health and inspire hope.

Our treatment philosophy is based on a model of recovery: The knowledge and belief that individuals, with appropriate treatment and support, can recover and/or manage their behavioral health conditions successfully. This model focuses on four major life dimensions:

  • Health: Overcoming or managing the symptoms or disease and making informed, healthy choices that support physical and emotional well-being.
  • Home: Having a stable and safe place to live.
  • Purpose: Engaging in meaningful daily activities, such as a job, school, family caretaking or creative activities, along with the independence, income and resources to participate in the community.
  • Community: Having relationships and social networks that provide support, friendship, love and hope.

Our services are tailored to the individual and built on clients’ strengths, talents, coping abilities, resources and values, including cultural beliefs. We address the whole person, recognizing the close relationship and interaction between medical and behavioral health, and the family and community systems in which they live.

We recognize that many individuals dealing with emotional, mental health and/or substance abuse issues have experienced trauma, and understand that current problems may be rooted in those experiences. That’s why we embrace Trauma-Informed Care, which asks not “what’s wrong with you?” but rather “what happened to you and how can we help?” Trauma-Informed Care helps the individual develop more effective strategies for coping with trauma and other difficult life experiences.

Our goal is to provide prompt access to appointments with licensed clinical professionals. We strive for excellent customer service and member satisfaction in all that we do.
