Housing

Transitional housing referrals and long-term stability planning so you’re not sleeping on someone’s couch indefinitely.

Substance use

Medication-assisted treatment (MAT), counseling, and peer support, covered by Medicaid and most major insurance plans.

Benefits & Nutrition

Applying for Medicaid, SNAP, and SSI. Replacing your ID or Social Security card if you left without it.

Employment

Job readiness, employer connections, and reentry-friendly opportunities that don’t hold a record against you.

Mental health

Individual therapy, psychiatric care, and crisis support, connected to the right level of care for where you are.

Common concerns

We’ve heard these before. Here’s the honest answer.

“I can’t afford treatment.”

Cost is not a barrier. CPIH accepts Medicaid, most major plans, and provides care regardless of ability to pay. We’ll help you get coverage if you don’t have it.

“I don’t have an ID or insurance.”

We start there. ID replacement and Medicaid enrollment are two of the first things we help with. You don’t need either one to reach out.

“Will my record affect my eligibility?”

No. CPIH specifically serves people coming out of incarceration. Your record does not disqualify you from any of our programs.

“I don’t want to end up in a system that reports me.”

We’re not law enforcement. We’re a treatment and support organization. What you share with us is confidential under HIPAA and standard healthcare privacy rules.

Does this cost anything?

No. All services through this program are free to participants. If you need help enrolling in insurance or benefits, we’ll help with that too.

Will my information be shared with anyone?

No. CPIH is a healthcare organization. What you share is protected under HIPAA and standard healthcare privacy rules. We are not connected to law enforcement or parole supervision.

Do I need to have ID or paperwork ready?

No. Reach out first, we’ll figure out documents together. ID replacement and benefits enrollment are two of the first things we help with.

What happens after I call or fill out the form?

A real coordinator, not a chatbot, will reach out within about a week. They’ll learn your situation and match you with the services that make sense for where you are.

I’m not sure what I need yet. Is that okay?

Completely. Most people don’t. That’s what the first conversation is for. You don’t need to have it figured out before you reach out.
