We serve individuals seeking primary care and/or behavioral health services who have Arizona Health Care Cost Containment System (AHCCCS), Medicare, commercial health plans and those who self-pay.
We provide primary care, psychiatry, therapy, case management, peer support, and residential and inpatient care to individuals experiencing mental health issues, including substance use disorder and crisis care services. We also serve veterans and active military.
Thoughtful care is given to individuals with mental illness whose physical illnesses complicate mental health treatment, such as diabetes, obesity, vitamin deficiencies, hypertension, poor diet, lack of exercise, cardiac disease and COPD. Individuals enrolled with Cenpatico Integrated Care or Mercy Maricopa Integrated Care may be eligible for supported employment, housing and peer services, as well as an intensive nine-week wellness program for managing chronic disease.
Eligible individuals (based on health plan benefits) have access to a nutritionist and pharmacist (at select locations), who aid in identifying the best course of treatments for each individual.
To best improve the physical health of members, we teach effective, culturally appropriate, and sustainable lifestyle interventions, including structured approaches that help people engage in physical activity, manage their weight, eat a more balanced and healthy diet, and pursue more healthy activities.
Sliding fee scale for essential services may be available to those in need, depending upon family size and income. Our focus is on the overall wellness of the individual.
