Eligibility and Insurance
- We serve individuals seeking primary care and/or behavioral health services who have Arizona Health Care Cost Containment System (AHCCCS), Medicare, commercial health plans and those who self-pay.
- We provide services to individuals experiencing mental health issues including Substance Use Disorder and crisis care services.
- We provide: primary care, psychiatry, therapy, case management, peer support and residential and inpatient care.
- We also serve veterans and active military personnel.
- Specialized care is given to individuals with mental illness who also have a physical illness, such as: diabetes, obesity, vitamin deficiencies, hypertension, poor diet, lack of exercise, cardiac disease and COPD, complicating their mental health treatment.
- Individuals enrolled with Cenpatico Integrated Care or Mercy Maricopa Integrated Care may be eligible for supported employment, housing and peer services, as well as an intensive nine-week wellness program for managing chronic disease.
- Based on health plan benefits, eligible individuals have access to a nutritionist and pharmacist (at select locations) who aid in identifying the best course of individual treatment.
- To best improve the physical health of members, we teach effective, culturally appropriate and sustainable lifestyle interventions that promote healthy physical activity, weight management and a more balanced and healthy diet.
- No one will be denied access to services due to inability to pay. There is a discounted/sliding fee schedule available based on family size and income.
- Our focus is on the overall wellness of the individual members.
